PUBLIC RECORDS REQUEST

Requests for Public Records may be completed online using the portal, or through any of the following points of contact. (Before submitting, please read our FAQs.

  Note:

  • If EMS Run Report is Requested, they need to Fill out the HIPPA Authorization Form
  • The only Forms of Identification to be accepted are a current Driver's License or Ohio State ID card with a Picture.


Payments can be made in person or mailed to 1 Business Park Rd. Dayton, Ohio, 45417. 

Checks should be out to the Jefferson Township Board of Trustees.
